Apple Home Healthcare has developed a series of signature programs that incorporate state-of-the-art technology and the latest research findings to ensure the highest quality care. Specialized health management programs to address the specific health conditions
Falls are the leading cause of nonfatal, unintentional injuries treated in a hospital ER department. Apple Home Healthcare’s Fall Prevention program focuses on reducing the risk of injury and falls for patients that are at risk or who have fallen in the past. Our team approach consists of nurses, physical therapists, and occupational therapists who develop a plan to improve the safety, balance, coordination, and ambulation of our at-risk patients. By preventing injuries and falls, our patients are able to safely remain at home.
Our Post-Operative Joint Replacement Care program is designed to help patients seamlessly transition from hospital to home so they can recover and rehabilitate comfortably following total hip or knee replacement surgery. RNs, PTs, and OTs work closely with orthopedic surgeons prescribed orders to maximize the patient’s function at home. Clinicians focus on a return to independence with a home safety assessment, excellent wound healing and pain management, and attainable rehabilitation goals.
At Apple Home Healthcare, we pride ourselves in practicing a holistic approach to wound care. Wound care can often be challenging due to the presence of other health conditions; successful treatment of difficult wounds requires assessment of the entire patient and not just the wound. Apple’s wound care nurses and physical therapists use evidence-based techniques to ensure that the best possible treatment is provided to decrease the healing time of wounds and likelihood of a hospital readmission.
Our Medical Social Workers (MSWs) work with the patients and families to keep patients at home in a safe and healthy environment. Medical Social Workers conduct assessments to help determine patient needs and identify any limitations, barriers, or safety issues that might impede recovery. Additionally, MSWs educate patients and families regarding community resources and support and assist with long range planning, including advance care planning and directives, and overall care opportunities.
At Apple Home Healthcare we understand that living with diabetes may be challenging. From establishing a healthy lifestyle to managing blood sugar and insulin levels, the disease may require extra care to allow individuals to stay independent. Apple Home Healthcare has experience in helping patients and their families live a quality life with diabetes. Our home healthcare professionals can visit people with diabetes in their homes or at senior living communities to provide care or supplement a healthcare regimen.
Our Psychiatric Home Health program offers psychiatric nursing services to help patients and their caregivers coping with depression, dementia, Alzheimer’s, schizophrenia and a variety of other psychiatric illnesses. Certified psychiatric nurses work closely with the patient, family, and physician or psychiatrist to develop a specific treatment plan to provide counseling and intervention, prevent hospitalizations, coordinate access to community mental health services, and give patients the tools they need to improve their quality of life.
Visual impairment is usually caused by eye diseases or health conditions, such as age-related Macular Degeneration, cataracts, Diabetes and Glaucoma. Macular Degeneration is an eye disease caused by degeneration of the cells of the macula lutea, resulting in blurred vision and sometimes blindness. Apple Home Healthcare’s Low Vision program is designed to help reduce limitations due to visual impairments. Our specially trained therapists help our low vision patients live more confidently, safely and independently.
For patients suffering from congestive heart failure, the cycle of re-hospitalization is frustrating and can take its toll on both patients and families. Apple’s Heart Failure Management program can help patients interrupt this cycle and manage their disease at home. The Heart Failure Management program teaches patients and their families how to manage their medication, diet, and activity to keep them feeling well. It also helps them identify symptoms early, so they can report to their physicians and possibly prevent a hospital visit.
Neuro-cognitive Rehabilitation is a multidisciplinary approach to address the entire spectrum of a neurocognitive disorder beginning at the onset of a cognitive and physical decline. Apple’s experienced psychiatric nurses, PTs, and OTs work with physicians and psychiatrists to provide care to address such conditions as Parkinson’s disease, ALS, stroke, Motor Speech Disorder, Multiple Sclerosis, dementia, and other neurocognitive disorders. Our clinicians make sure the needs of the patient and caregivers are managed effectively.
Apple Home Healthcare offers a unique Tele-monitoring program to help patients maintain a healthier lifestyle, stay independent, and reduce Emergency Department visits and hospital admissions. Using advanced equipment, the program monitors vital signs and can catch an early warning sign, such as a sudden rise in blood pressure, and treat it before it becomes a bigger problem. The information is sent to an Apple clinician, who can promptly determine if follow-up care or actions are needed.
