Our team of nursing home care professionals are focused on avoiding unnecessary hospital stays so our patients can recover and thrive at home. At the start of care, we perform a thorough risk assessment to determine an appropriate care plan and communicate it with the patient, their physician, and family caregivers. Our RNs continually assess and evaluate patients, coordinate care, supervise home care team members and work with physicians.

Our nurses have specialty training to care for various types of diagnoses and medical conditions.All of our nurses have bachelors’, masters’ or other advanced degrees; have passed rigorous national licensing examinations; and have met additional
state requirements.

Additionally, we select clinicians who demonstrate qualities that make them ideal for serving the unique health care needs of seniors and others, including patience and exceptional compassion.

Apple Home Healthcare’s RNs and therapists work with patients and their family members to provide care at times that are convenient and respectful of our patients’ schedules.

Our highly skilled RNs provide specialized care and work closely with patients’ physicians, family members and caregivers to keep patients well, and prevent illness and injury. Our nurses have specialized training and certifications to address all of your specific healthcare needs.
Nursing Care Primary Duties:
When our patients need assistance to manage, observe, and evaluate their recovery progress, they can trust our RNs to deliver quality care.

 

  • Assessing the patient’s condition
  • Developing and implementing a unique care plan
  • Administering complex treatment and medications
  • Working with your pharmacy on any medication changes
  • Updating your primary physician about any health changes
  • Directing other members of the home care team
  • Monitoring vital signs, body functions and health status
  • Cleaning, observing and redressing wounds
  • Performing injections and other hands-on treatments
  • Checking and helping with catheters and feeding tubes
  • Providing nail, foot and skin care
  • Teaching basic tasks of care to family members

Apple Home Healthcare’s psychiatric-mental health registered nurses (PMHN) work with patients to assess their mental health needs, develop a nursing diagnosis and plan of care, implement the nursing process, and evaluate it for effectiveness.

Our chemotherapy nurses are responsible for assessing patients pre-, during and post-chemotherapy. The chemotherapy nurse is able to understand pathology results and their implications, and have an in-depth knowledge of the expected side effects of chemotherapy treatments.

Our medical and surgical nurses have experience in caring for both the physical and psychosocial health needs of patients who have undergone surgical procedures.

Apple Home Healthcare’s Certified Orthopedic Nurses focus on the prevention and treatment of musculoskeletal disorders. Orthopedic nurses have specialized skills such as neurovascular status monitoring, traction, continuous passive motion therapy, casting, and care of patients with external fixation.

Apple Home Healthcare’s anticoagulation nurse is responsible for monitoring patients’ anticoagulant therapy, such as Coumadin, based on clinical standards and protocols. Other responsibilities include anticoagulant medication administration, point-of-care testing, health education to patients and families regarding instructions for medication and care related to anticoagulation therapy.
